How to order and pay for INCARE?

Ordering services for oneself

You can make an appointment by emailing incare@incare.cz or by filling-in the contact form from our web contact page and we will get back to you. You can also call us directly on 273 162 162.

For the annual care programme you can pay by cash, credit card or bank transfer. One-time preventive packages must be paid in advance. You can also pay for the packages using your Caffeteria Benefit Plus, Benefity.cz., Edenred or Sodexo account.

Ordering services for your family or friends

If you want to order a package for your family or friends, the procedure is the same as if ordering a package for yourself. If you are ordering a package already for a specific date, you will be asked to fill in the name of the recipient, his/her birth registration/insurance number, place of residence, email, phone number and the name of the insurance company.

Ordering services as a company gift or benefit

We treat each of our clients individually. This also applies to corporate clients. If you are interested, please contact us and one of our programme coordinators will come back to you with more information.
 

  Ordering, payment and delivery of a gift voucher

We offer our clients gift vouchers of any value, which you can donate to your family and friends. If you are interested, please contact us and one of our programme coordinators will come back to you with more information.

 

How do I pay?

1) bank transfer

One-time packages:
Bank name: UniCredit Bank

Variable symbol: The programme coordinator will give you the details
Payment must be credited to the account before the first INCARE service is provided.

Annual care:
Bank name: UniCredit Bank
Account no.: 2112736548/2700
Variable symbol: contract number
Payment must be credited to the account before the first INCARE service is provided.

2) in cash on site

Pre-payment is required prior to the completion of the one-time package. In the case of the annual INCARE program, at the time of signing the contract.

3) by card on site

Pre-payment is required before completing the one-time package. In the case of the annual INCARE programme, at the time of signing the contract.

4) by drawing from BENEFIT Plus, Benefity.cz, EDENRED, SODEXO UP CZ (eBenefity, Gallery Beta, Unišek, Unišek+, FKSP, Cadhoc) account

On the actual date of examination, however yet before the initial examination is carried out. Can only be used for one-time packages.*

*Conditions for application
When ordering a package, the client must inform that payment will be made through Benefits. The client has a valid Benefits card, which will be presented upon arrival at INCARE. To order a package through our website, simply write in the message on the order form that you will be using the Benefit card payment.
